Client Info and Travel Itinerary Form

Chow time! How much food does your pup get per meal? Any additive to their food? Special instructions? I feed twice daily morning and evening. Please remember to include 2-3 days worth of extra food, incase you are delayed or other emergency.

Travel Plans

If you are traveling out of the country or to a remote location make sure you have an emergency plan for you pup? I.E. Vet info current, local emergency contact that knows they are your pups emergency contact. Have you notified your vet and made Kendra Luck a proxy caregiver for your dog? Many vets will allow you to leave a credit card on file for when you travel.

 

 What to bring and what to leave at home when your dog comes to SSS:

  • Food, with dog’s name and meal amount on container, plus 2-3 days extra food, in case of travel delays.
  • No bowls are necessary; I’ve got ’em.
  • Dog toys, of course I have lots, but if you decide to send a toy with your pup know that there are other dogs here and toys may get destroyed or lost, I will do my best to keep track of them and return them to you, but no promises.
  • Dog bed, I have lots of dog bed here, SO unless your dog is really attached or has a special needs bed I.E. old dog with extra thick memory foam bed , etc.
  • Leash and harness (if your dog uses one)
  • Have current flea treatment. I work really hard to have a flea-free environment and that is only possible by having relatively clean, flea free and treated dogs.
